Through proactive planning, personalized wellness support, and trusted care coordination, members gain peace of mind knowing they have a plan in place long before care is needed — allowing them to remain independent and prepared for the future.

What is a Continuing Care at Home Program?
Continuing Care at Home (CCaH) program is for independent adults who want to age in place, preserve assets, and thrive in their retirement years with the peace of mind that their needs will be met. CCaH programs also have separate events for members to provide engagement and socialization. As a member of ThriveMore at Home, you'll receive a lifetime promise of care and services. ThriveMore at Home offers the support you need to stay in your home while providing expert service and a proactive approach to aging at home.
What Is a Wellness Coordinator?
Our Wellness Coordinators use their knowledge and expertise to help members take control of their future by designing a plan to maintain and promote wellness. As a result, if a member experiences an injury or illness, Wellness Coordinators are able to support them every step of the way and serve as an advocate for them and their family.
Wellness is our priority. Our Wellness Coordinator works with members to strategize the best plan possible to maintain their overall health. Additionally, members have access to the Wellness Coordinator when/if needed to engage with care service providers and navigate health systems.

No. ThriveMore at Home is not a home care agency. Home care typically provides caregivers when you already need daily help. ThriveMore at Home provides ongoing guidance, care coordination, and advocacy so you have a plan in place long before care is needed — and trusted support to coordinate services when it is. Think of ThriveMore at Home as a partner for aging well at home, not just a service provider.
ThriveMore at Home is ideal for older adults who want to remain in their own home as they age, individuals or couples who are currently independent and healthy, and people who want peace of mind and a clear plan for the future. You do not need to require care today to join. Members join while they are healthy so decisions don’t have to be made during an emergency.
Membership includes: a dedicated RN Wellness Coordinator who gets to know you personally; ongoing wellness check-ins and proactive planning; care coordination and advocacy during illness, hospitalization, or recovery; guidance navigating healthcare systems and providers; support with future care planning, including long-term care needs; trusted oversight so you and your family are never alone in decision-making; access to an emergency pendant system; and access to our concierge partner, Dorvie, for assistance with home repairs, transportation, food delivery, and other day-to-day needs. Services are personalized — there is no one-size-fits-all approach.
Your Wellness Coordinator is your primary point of contact and trusted guide. They build a relationship with you over time, help you plan for aging well, and step in to coordinate care and support if your health changes. They also advocate for you and your family, helping reduce stress during challenging moments.
ThriveMore at Home does not directly employ caregivers for daily hands-on care. Instead, the program coordinates and oversees care when needed — helping identify appropriate services, manage transitions, and ensure quality and continuity.
If a health event occurs — such as a fall, illness, or hospitalization — your ThriveMore at Home team helps coordinate next steps, communicate with providers, and support you and your family through the process. Members often share that having someone who already knows them makes a stressful situation far more manageable.
Yes. ThriveMore at Home is designed to support both members and their families. Adult children often tell us the program gives them peace of mind knowing their parent has professional support and advocacy — and that they are not solely responsible for managing complex decisions during a crisis.
ThriveMore at Home currently serves members in North Carolina, with a focus on the Triangle and surrounding areas. We currently have members in Raleigh, Durham, Chapel Hill, and Cary. Availability may vary, so the best way to confirm eligibility is to speak directly with the team.
Yes. ThriveMore at Home is part of ThriveMore, a trusted, faith-based, nonprofit organization that has served older adults in North Carolina for more than 75 years. The program was created to extend ThriveMore’s legacy of care beyond campus communities and into people’s homes.
